2021 COVID-19 Vaccine/Flu Shot at CISD Home Football Games

CISD is partnering with Tom Thumb to offer a COVID and flu vaccination clinic at each Varsity home football game this season. The vaccine clinics will be held at Buddy Echols Field on the home side of the stadium from 6 -7:30 p.m.

In order to receive a vaccine, fill out the form below, and then fill out the consent form at go.coppellisd.com/VaccineConsentForm and bring it to the clinic along with your insurance cards. The COVID vaccine can be provided to those 12 years old and older, while the flu vaccine can be provided to anyone ages 7 and older. Please fill out a form for each person getting a vaccine.
